Registration fee includes breakfast on October 22nd and 23rd. Lunch will be on your own.
All fees are due on or before the first day of the PBIS Leadership Forum. We accept credit cards, checks, money orders, and purchase orders. All payments must be in US dollars and should be made payable to Midwest PBIS Network. Please submit your check, money order, or Purchase Order payment to:

Invoices
Invoices will be automatically generated at the conclusion of registration and may be displayed, downloaded, printed, or shared via email by clicking the "Receipt/Invoice" button after your registration has been submitted. Invoices may be accessed at any time after registration by clicking the link in your registration confirmation email. When accessing from the confirmation email, you will be required to enter your registration confirmation number.
IMPORTANT NOTE: Please make sure you submit the invoice to your business office for payment. No copies of your invoice will be sent via email or US mail. Billing information collected during registration will be used to send balance due statements on unpaid registrations only.
Substitutions
Substitutions/registration transfers can be made onsite at the PBIS Leadership Forum Information Desk.
Changing your session selections
If you would like to make changes to the sessions you may do so in our conference app. You must first unregister from your current registered session, and then register for the new one you'd like to attend, if it's available (see note below).
Locate the Agenda icon on your app homescreen
Tap 'My Agenda' at the top of the page
Find the session you no longer want to attend
Click on the UNREGISTER button under the session title
Locate the session you would like to attend under 'Agenda' at the top of the page
Click on the REGISTER button under the session title
NOTE: if the session is full, there will not be a REGISTER button available
Cancellation Policy
Cancellations received in writing via email (support@midwestpbis.org) or via mail to 1755 Park Street, Suite 200, Naperville, IL 60563 by October 10, 2025, will be refunded minus a 5% registration service charge to cover non-refundable expenses to the Midwest PBIS Network. Please note that if you do not cancel and do not attend, you are still responsible for payment. Substitutions may be made at any time.
Purchase Orders
Make sure all staff have registered online prior to submitting your PO. We are not able to enter registrations from a Purchase Order.
Be sure to include the forum name, date, and the names of all registrants on your PO.

Credit Card Payments*
*The online payment option is currently closed but will again be available when the registration system opens back up on October 21, 2025
You may pay your registration fees by credit card when you register using our online registration system. If you would like to pay by credit card after your registration has been completed, you will need to log in to your registration record to make a payment online. See below for instructions on how to access your registration record to make a credit card payment. Credit card information can not be accepted via email, mail, or telephone.
NOTE: You will need the confirmation number and email address of the registrant to access the registration system. (Use the info for the first person listed if you are paying the balance due for a group.)
